The Providers and Staff of Chicago Cosmetic Surgery and Dermatology would like to welcome you to our Practice. We are looking forward to meeting you at your upcoming appointment. The following information will assist you with the registration process. In order to better facilitate information for our practice and insurance companies, it is necessary to complete the office forms and bring them to your appointment.
WHAT PATIENT FORMS DO I NEED TO COMPLETE?
For your convenience our patient Registration Form, Medical History Form and Financial Policy are all writable PDF’s that can be found online. To ensure a timely visit, you can complete them in advance of your appointment, digitally sign, save and email to [email protected]. You can also fax them to our office at 312-245-9964. By completing our forms in advance, you will save a significant amount of time during your in-office visit.
WHAT DO I BRING TO THE APPOINTMENT?
Please be prepared to bring your pre-completed patient forms to our office along with your current medical insurance card and a form photo identification. If your insurance requires a referral, please bring the referral form with you. For measures of security, the desk staff at 20 W. Kinzie will also require to see your photo identification when you enter the building – please have this ready to show them otherwise it may delay your appointment.
SHOULD I ARRIVE EARLY?
We recommend arriving 15 minutes prior to your first appointment so as to allow sufficient time for the registration process. If you would prefer to complete the forms at our office, please arrive 30 minutes prior to your appointment time.
WHERE ARE YOU LOCATED?
Our office is located in the Dearborn Plaza Building at 20 West Kinzie Street. Please be prepared to check in with the security desk in the lobby and show a photo ID. We recommend parking at the Hertz AMA Parking Lot located on State and Kinzie, 401 N State St, which costs $15 for 12 hours. We can also suggest valet parking in and around the area from $14 or by downloading a popular mobile phone app, like SpotHero or ParkWhiz, to find and book parking in advance.
WHAT LANGUAGES ARE SPOKEN AT YOUR OFFICE?
The spoken languages at our facility among staff and/or physicians are Spanish, Hindi, Greek, Macedonian, Arabic, Serbian, and French. If you require assistance with translation please let us know in advance of your appointment so we can schedule you appropriately.
WHAT ABOUT PAYMENT?
Be aware that we are considered specialist providers. We will collect any specialist copay (usually documented on the front of your insurance card) and estimated remaining balance, not covered through insurance, at the time of your visit. All self pay and cosmetic patients will be expected to pay in full at the time of any services rendered. We do collect deposits for certain procedures and treatments.
